Cost:

As of December 18, 2012, $38.45 nonrefundable fee for the first record, and $15.00 for each additional copy of the same record ordered at the same time. $20.00 for a different record as part of a group order. Fees for rush delivery by UPS or Express Mail vary by vendor, time and place.

Payment to:

All major company credit or debit cards. Most prepaid debit cards accepted. Government issued benefit cards are not accepted.

Requirements:
  • Complete record information.
  • Applicant’s Social Security Number and date of birth required to generate an identity screening quiz.
  • Scanned or faxed copy of valid ID or alternative ID may be required. VitalChek fax number: 1-888-825-3805.
  • In some cases, power of attorney, proof of legal guardianship, proof of change of name, or proof of relationship may be required.
Processing Time:

Within 2 business days after receipt unless a record problem is discovered or an amendment is requested. Oregon Vital Records processes orders Monday through Friday, excluding holidays and designated furlough days. Orders placed by phone or on the web through VitalChek are forwarded to Oregon Vital Records within 24 hours once orders are complete.

 

The non-refundable fee for one copy is $38.45. This fee includes a $8.95 vendor fee, $5.50 quick processing fee, $4.00 identity authentication fee, and the $20.00 record search fee.

You will be requested to provide your Social Security Number (SSN) and date of birth to generate a quiz to screen your identity. If you fail the identity quiz, applicants must scan and email or fax in a photocopy of their current valid ID or alternative identifying documents. When requested, fax your ID to 1-888-825-3805. Once a search for a record has been started, orders cannot be canceled, nor fees refunded.

Orders may be delayed for missing or wrong record information on the application, if the applicant ordering is not eligible, or if records are not yet registered. Corrections or amendments to the record will add additional time for processing.

Rush Delivery

The non-refundable $38.45 fee for one record covers rush processing and shipping by regular mail within two work days unless a record problem is discovered.

VitalChek will forward orders within 24 hours after all information is provided and any documentation requirements are complete. Overnight delivery by UPS or Express Mail is available for an additional fee that will vary by vendor, delivery time, and place. Fees for delivery outside the continental United States are higher. Delivery of a record using an overnight delivery service takes from two to five days. For example, an order forwarded from VitalChek on Thursday could be processed and sent by UPS on Friday for delivery on Monday in the continental United States. The fee for this delivery would be an additional $20.
