Consumer Help: Ordering By Mail 

Sending a Request by Mail: you must include an order form, complete vital record information and a photocopy of your driver's license or official ID. Send your order to Oregon Vital Records, PO Box 14050, Portland, OR 97293-0050. Please allow three to five weeks for processing and delivery. The non-refundable fee for one copy is $20 and each additional copy of the same record ordered at the same time is $15. Checks and money orders may be made payable to OHA/Vital Records. Processing of orders may be delayed by three weeks if payment is by check and the check is for a new bank account with a low check number or is made with a temporary check, the check is from an unrelated third party, or the printed name or address shown on  the check has been altered. RUSH ORDERING

Mail Orders: To speed processing, send your request by any overnight delivery service to Oregon Vital Records, 800 NE Oregon Street, Suite 205, Portland OR 97232. Orders received by priority mail or overnight delivery are usually sent out by regular mail either the same day or the next working day assuming no record problems are discovered. If you need the order to be returned by overnight delivery, please enclose a pre-addressed, prepaid overnight delivery envelope. Processing of orders may be delayed by three weeks if payment is by check and the check is for a new bank account with a low check number or is made with a temporary check, the check is from an unrelated third party, or the printed name or address on the check has been altered. We may not be able to process your request if you are not eligible to order, the record event is so recent that we have not received the record, your order information does not match what is on the record, or you do not have valid ID.
