How to order:

On the web at www.vitalchek.com at any time.

Cost:

$36.50 nonrefundable fee for the first record and $15.00 for each additional copy of the same record ordered at the same time. $20.00 for a different record as part of a group order. Fees for rush delivery by UPS or Express Mail vary by vendor, time, and place; see below for details.

Payment to:

All major company credit or debit cards. Most prepaid debit cards accepted. Government issued benefit cards are not accepted.

Requirements:
  • Complete record information.
  • Applicant’s Social Security number and date of birth required to generate an identity screening quiz.
  • Scanned or faxed copy of valid ID or alternative ID may be required. VitalChek fax number: 1-888-825-3805.
  • In some cases, power of attorney, proof of legal guardianship, proof of change of name, or proof of relationship may be required.
Processing Time:

Within 2 business days (Monday through Friday), unless a record problem is discovered or an amendment is requested.


The non-refundable fee for one copy is $36.50. This fee includes a $7.00 vendor fee, $5.50 quick processing fee, $4.00 identity authentication fee, and the $20.00 record search fee.

You will be requested to enter your full Social Security number (SSN) and date of birth to generate a quiz to screen your identity. If you fail the identity check, applicants must provide a copy of current valid ID or alternative identifying documents. When requested, fax your ID to 1-888-825-3805. Once a search for a record has been started, orders cannot be canceled, nor fees refunded.

VitalChek will forward orders within 24 hours after all the information is provided and any documentation requirements are complete. For information on special overnight delivery options, see Rush Delivery options below. Orders may also be delayed for missing or wrong record information on the application, if the applicant ordering is not eligible, or if records are not yet registered. Corrections or amendments to the record will add additional time for processing.

Rush Delivery

The non-refundable $36.50 fee for one record covers rush processing and shipping by regular mail within two work days unless a record problem is discovered. Overnight delivery by UPS or by Express Mail is optional for an additional fee. Fee will vary by vendor, delivery time and date, and place. Fees for delivery outside the continental United States are higher. Delivery of a record using an overnight delivery service takes from two to five days. For example, an order forwarded by VitalChek on Thursday could be processed and sent by UPS on Friday for delivery on Monday in the continental United States. The fee for this delivery would be an additional $20.

Other Vendors on the Internet

If you use another vendor on the Internet, you will pay additional fees without receiving fast and efficient processing of your record request. Other vendors may provide order forms only and instruct you to mail them to our office, or they may mail in your order with the $20 record fee and the photocopy of your ID. These orders will be processed in five to seven weeks with regular mail orders. The Oregon Vital Records office has no agreements with any vendors except the VitalChek Network. We have no control over what fees other web vendors charge, nor can we guarantee the confidentiality of any vital record data provided to them. If you want to cancel an order placed with these vendors or receive a refund, you must call the vendor directly or contest payment through your credit card company. If you believe the vendor has failed to provide you with a record or services, you also may report the problem to the consumer protection agency in your state.
