Oregon Vital Records Order Forms

Mail Order Requirements

If you mail in a request for a vital record, you must include:
We require this in order to screen for fraud. Knowing why someone needs the record helps us review requests for a large number of records, determine whether the applicant has a right to order the record, and know whether to send a short or long form certificate. Having the applicant's signature ensures that records are being requested by an eligible person.
Read the last page of the order form for ordering instructions and suggestions for alternative ID when the applicant does not have a current valid driver's license or passport.

You may need a full image or "long form" birth certificate. Full image records are available for births that occurred from 1903-2007.
