The State Vital Records office has updated our birth and death order forms.
Beginning August 1, 2011 you may order these new forms from the State Vital Records Office, or you may print them from this web page. Customers may begin to use the new forms immediately but we will still accept old order forms through September 15, 2011. On September 16, 2011 we will reject old forms unless they include the new data items.
The new order form requires three new things:
● A photocopy of the applicant’s ID (or alternative documents)
● The applicant’s reason for needing the record; and
● The signature of the applicant for the record.
The biggest change is the requirement for a photocopy of the applicant’s ID or alternative documents to prove the identity of the applicant. We are making this change to screen more thoroughly for fraud. Knowing the reason for ordering will help us when we receive a request for a large number of birth records or to determine whether to send a short or long form certificate. Having applicant signatures will ensure that records are being requested by eligible persons. Please notice that the back of each form has ordering information as well as suggestions for alternative ID when the applicant does not have a current valid driver’s license.
