Living Well with Chronic Conditions
A Six-Week Workshop

Living Well is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, participants get support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.

The workshop teaches real-life skills for living a full, healthy life with a chronic condition.

  • Classes are fun and interactive.
  • Participants share their successes and build a common source of support.
  • The workshop builds confidence around managing health, staying active, and enjoying life.

Related Programs

Other chronic disease self-management programs supported by the Oregon Health Authority and the Oregon Department of Human Services include Tomando Control de su Salud, the Spanish-language, culturally appropriate version of Living Well, and the Positive Self-Management Program (PSMP), a workshop for people living with HIV/AIDS.
