Forms are updated from time to time, so rather than stockpiling them in your file cabinet until they are out of date, just download and print when needed, or better yet, you can use Orpheus directly.
Orpheus, the Oregon Public Health Epidemiology User System, is the byproduct of an epidemiologic effort in integrating Oregon's reportable communicable diseases for state and local use, into a single application. Orpheus exemplifies health information exchange (HIE), as most case investigations are initiated by the electronic laboratory reports (ELRs), which are automatically imported and accessible to both local and state users, who can work together on cases. Orpheus uses a commercial off-the-shelf application (FileMaker Pro), is compatible with national standards, and leverages the state's enterprise information architecture, such as the Citrix access gateway, to assure that it complies with the highest level of security and confidentiality.

For reportable diseases lacking Oregon-specific investigative guidelines or case report forms, please contact the epidemiologist on call for assistance.
Please fax the first part of the form (demographic information) to ACD as soon as possible, even if you have not yet completed case investigation.
