Tracking Chronic Conditions and Their Risk Factors in Oregon

The Oregon Public Health Division, Health Promotion and Chronic Disease Prevention (HPCDP) section reports data on chronic conditions and specific risk factors associated with chronic conditions. This information helps guide Oregon’s efforts to prevent and help people manage chronic conditions and reduce health inequities among populations that are most affected.
