Informed Consent 

With few exceptions, Oregon Statute (433.045) requires that informed consent be obtained before submitting a person's blood for HIV testing in Oregon.

Oregon Administrative Rules (333-012-0265) dictate that the consent be delivered orally and in writing "...as specified in Appendix 1." In addition the individual being tested must sign a copy of the consent form, and the signed copy of the form is to be retained for at least seven years by the party submitting the specimen. Appendix 1 consists of a  model consent form (pdf).  Alternative versions may by used if they are in substantially equivalent form.
