All tanning devices must be registered with the State of Oregon Radiation Protection Services Tanning Bed Program, OAR 333-119-0020(1).

Once the application is received and the license fee has been paid a license is issued for each tanning device. Licenses are renewed every year on January 1. The registration fee is $100 for each tanning device, failure to pay registration fees will subject the facility to civil penalties as outlined in OAR 333-124. 

Keep in mind:
Licenses cannot be transferred from one person to another person.
Licenses cannot be transferred from one tanning facility to another tanning facility.
Licenses cannot be transferred from one tanning device to another tanning device OAR 333-119-0020(9).
