Work Group Charge and Logic Model

  • Define clinical elements from the 3/17/10 developed framework as a component of the Oregon Home Visiting System with sufficient detail to contribute to Oregon’s application for the Home Visitation Grant Program (HR 3590).  
  • Define the clinical prenatal and postpartum outcomes for the target population: Oregon’s non-NFP eligible clients that meet the new Public Health home visiting requirements: low income, high risk factors, first and non-first births.
  • Complete activities as described in the Logic Model and present the proposed recommendations to state and local MCH partners at the July 12th home visiting meeting.
