The Oregon Family Planning Program Manual is divided into six sections. Clinics may need some or all of them, depending on whether they are CCare-only or CCare and Title X, and if they participate in the IPP project.

For your convenience the consent to sterilization forms are provided here.
All other forms for vasectomies are exclusively for Title X and located in Section A Exhibits 5 and 6.
