Actively growing isolated organism, in pure culture, on an agar slant or plate media, or appropriately collected  swab specimen (in appropriate transport tube) from nasopharynx (respiratory diphtheria) or cutaneous wound (wound diphtheria).

Seal tube or plate and place isolate or swab transport media into biohazard bag. Refrigerate or hold at room temperature. Do not freeze. Submit specimen according to Infectious Substance Shipping Guidelines.
