How to Order	Check the appropriate box, under MYCOBACTERIUM, on the General Microbiology Request Form included in the Collection Kit.

Specimen Collection Details
Collection	Obtain a QFT collection kit from the laboratory and draw 1ml of blood into each of the 3 specimen tubes. Shake each tube vigorously for 5 seconds.
Optimal Volume	1.0 ml per tube (1.0 ml line is printed on tube label)
Minimum Volume	0.8 ml (max 1.2 ml)
Handling	Maintain all tubes at room temperature. Do not centrifuge.
Transport	Tubes must be sent at room temperature and be received in the laboratory within 16 hours of collection, M - F, between 8:00AM and 5:00PM.
Rejection Criteria	Unlabeled specimens, specimens not submitted as per collection instructions or without completed requisition forms will not be processed.
