How to Order

Check “Other Sendout For” under Miscellaneous Serology on the Virology/Immunology Request (form 42) and write in “Yersinia Antibody.”

Also complete the CDC Specimen Submission Information form located at: public.health.oregon.gov/LaboratoryServices/Pages/CDC-Testing.aspx.

All clients; Requires completed CDC Form 50.34, including date of onset of symptoms
