MALHEUR COUNTY – Outreach and Enrollment Assister - Malheur County HEALTH DEPARTMENT - Ontario, OR

MARION COUNTY – Health Team Supervisor – Public Health Nursing – Marion County Health Department, Salem, OR

MCHD has an interest in developing a nurse manager with an interest in leadership and a passion for public health. The Health Team Supervisor will supervise a multi-disciplinary team of nurses, paraprofessionals and health educators.  The position is part of a team of six public health supervisors.  The programs supervised will be nursing-focused, but the final assignment may vary depending upon th applicant's prior experience and interest.

MARION COUNTY – Public Health Worker (PHW) 1, Bilingual – Marion County Health Department, Salem, OR

MULTNOMAH COUNTY – Nursing Supervisor, Maternal Child and Family Health – Multnomah County Health Department - Portland, OR
Maternal Child and Family Health (MCFH) is a unit within the Public Health Division of the Multnomah County Health Department with multiple home visiting programs. These programs use community based strategies to ensure that diverse populations in Multnomah County have access to services that best serve their communities. MCFH programs ensure a trauma informed, equity focused systems approach to Multnomah County home visiting resources.Toxic stress is now understood as one of the fundamental influences for early childhood and adult health. The primary goals of MCFH programs are to improve pregnancy outcomes, decrease health inequities among people of color, improve child health and development, and improve economic self-sufficiency of the family through the interruption of toxic stress and the building of resiliency.

MULTNOMAH COUNTY – Licensed Practical Nurse (LPN) Bilingual Spanish – Multnomah County Health Department – Portland, OR
The Panel Manager tracks the health of the population of patients assigned to a provider panel. This includes proactively tracking prevention activities, screening and chronic disease monitoring and providing patient education. The LPN provides scrubbing for upcoming provider appointments and participates in huddles. Manages team communication including answering team phone, addressing messages and in-basket as well as patient communication and follow-up. Submits refill requests and prior authorizations to providers in a timely manner. Performs general provider support duties as needed to support clinic flow including rooming patients, ordering labs as directed, giving immunizations, and medications as needed. Assists with assessment of walk-ins as necessary.
