The Stanford Chronic Disease Self-Management Programs, Living Well with Chronic Conditions and Tomando Control de su Salud, have been offered in Oregon for several years. Increasingly, organizations across the state have been bringing these programs to diverse audiences, including people with mental health conditions and low income populations, just to name a few. Learn about the applicability of Living Well and Tomando Control to diverse populations in Oregon and some of the results of these efforts.
