Nonmedical Vaccine Exemptions

About vaccine exemptions

By far, most parents in Oregon vaccinate their children. But parents who do not want their children to be vaccinated can claim a nonmedical exemption for one or all school immunizations. Oregon college students can claim a nonmedical exemption for the measles vaccine (pdf).


How to claim a nonmedical exemption

There are two ways to claim this exemption. You can talk to your health care provider and get a Vaccine Education Certificate. Or, you can watch an online education module and print a Vaccine Education Certificate. The process for college students is a little different than the process for parents. Please read the instructions below or view this flowchart (pdf).

1. Talk to your health care provider.

Parents:
  1. After talking with your health care provider, s/he can give you a signed Vaccine Education Certificate.
  2. Fill out the nonmedical exemption section of the Certificate of Immunization Status (pdf).
  3. Turn in both forms to your child's school or child care.
College students:
  1. After talking with your health care provider, s/he can give you a signed Vaccine Education Certificate.
  2. Turn in this form to your college or university.

OR:

2. Watch an online education module.

Parents:
  1. Watch the Vaccine Education Module for parents and print out the Vaccine Education Certificate at the end of the module.
  2. Fill out the nonmedical exemption section of the Certificate of Immunization Status (pdf).
  3. Turn in both forms to your child's school or child care.
College students:
  1. Watch the College Measles Module.
  2. Print out and sign your Vaccine Education Certificate at the end of the module.
  3. Turn in this form to your college or university.

For parents: Vaccine Education Module

IMPORTANT! Read before you start the module:

To protect your privacy, we do not store any information you enter. For this reason, you will need to start over if you close the module before you are finished. If you need to take a break, leave the module window open. When you reach the end, make sure you print your Vaccine Education Certificate before leaving that screen. You will not be able to go back and print the certificate later.

Tips:
  • Allow 15 to 60 minutes to finish this module. The amount of time depends on the number of vaccines for which you are getting an exemption.
  • Make sure your computer or device is connected to a printer before you begin.
  • This module has audio. Use headphones or make sure your speakers are turned on.
  • This module takes a few moments to load when you first start. Please be patient.
