For clinics new to VFC:
If you would like to enroll your clinic in the
Oregon Vaccines for Children program, follow these steps:
Step One:
Call the Oregon Immunization Program at (971) 673-0300 and request a VFC Recruitment Packet or download the packet documents below.
Private Providers:
Public Providers
These are clinics run by a Local Health Department (LHD) or clinics partnering with the an LHD by written contract.
Step Two:
Complete and sign the following forms from the recruitment packet:
- Provider Enrollment Form
- Provider Agreement
- ALERT IIS Agreement, if you do not already have a current agreement on file with ALERT IIS
Mail or fax all completed forms back to the Oregon VFC Program

Step Three:
Schedule a pre-enrollment clinic visit with the VFC Health Educator assigned to your county by calling (971) 673-0300. All new clinics must complete a pre-enrollment visit before enrollment into VFC will be approved or receipt of any VFC vaccine.
