Oregon Immunization ALERT
Alert logo

"The ALERT IIS is an excellent example of a successful public/private partnership that benefits the entire state. I hope everyone will use it well."

- Mel Kohn, MD, MPH, director of Oregon Public Health

The ALERT Immunization Information System (IIS) is a new, improved statewide immunization registry developed to achieve complete and timely immunization of all Oregonians. In 2010-11, the statewide ALERT registry system is merging with IRIS, the immunization records system that most local public health departments use, to create ALERT IIS. This new registry collects immunization data from both public and private health care providers to create complete records for individuals in Oregon. ALERT IIS offers many new features that will save time and provide valuable information in clinical and school settings. Authorized users can access immunization information on ALERT IIS via the new ALERT IIS tab on the current  ALERT web page, navigate to the new ALERT IIS webpage at www.alertiis.org, or through the toll-free Helpdesk line at 1-800-980-9431 (Portland Metro area: 971-673-0275)

Although the state immunization registry was originally focused on consolidating shot records for Oregonians ages 0 - 18 years, 2008 legislation allowed ALERT IIS to expand to a lifespan registry. ALERT IIS is now accepting data for all ages.
