Oregon Trauma Hospital Contact Update Information

The Trauma hospital contact list is an important tool for the Oregon Trauma System personnel to enable them to have current contact information on each trauma center.  Please have the trauma coordinator or their designee maintain this list with current information and make updates when changes occur in your institution.  It is only required to update the relevant field except your Hospital Name field.
