The Oregon Vital Records office, known as the Center for Health Statistics (CHS), is where you can get certified copies of birth and death certificates, as well as other vital records in Oregon.
Oregon law requires all vital events such as birth, marriage, divorce, registered domestic partnership, dissolution of registered domestic partnership, fetal death and death be permanently recorded and registered. These are legal records documenting a vital event.
Certified copies of vital records are required to get social security benefits, a driver’s license, passport or other legal documents.
There are more than six million vital records filed at the state. Birth and death records have been filed with the state since 1903. Marriage records have been filed since 1906, divorce records since 1925, and fetal death records since 1919. Information from these vital events and other surveys is used as a source of data for informing public health policy and planning throughout the state and country.
