Annual Statewide Forum

The Living Well with Chronic Conditions Statewide Forum is an annual gathering of organizations and individuals who are supporting those living with chronic conditions through Stanford's Chronic Disease Self-Management Program (CDSMP), which includes Tomando Control de su Salud and Positive Self-management Program (PSMP).

In many areas of Oregon the CDSMP programs are known as "Living Well with Chronic Conditions".

2012 Forum Summary

The seventh annual Living Well Forum took place on March 7, 2012 in Eugene. The keynote presentation was given by Dr. Kate Lorig, director of the Stanford Patient Education Research Center and developer of the Stanford Chronic Disease Self-Management Program.

The Living Well Forum also included a series of topic-specific breakout sessions applicable to program leaders, Master Trainers, coordinators, partners, and health care providers; such as financial sustainability, quality assurance, marketing, and how to build connections with health systems.

2011 Forum Summary

The Living Well Forum was opened to Living Well, Tomando Control, and Positive Self-Management Program leaders, Master Trainers, and program coordinators, Healthy Communities and Tobacco Prevention Education Program coordinators, and other partner organizations with an interest in Stanford’s Chronic Disease Self-Management Programs. 

The Forum included a plenary presentation by Teresa Brady, PhD, Behavioral Scientist, Centers for Disease Control and Prevention. The Living Well Forum also included three series of breakout sessions focused on topics like marketing, quality assurance, health literacy, financial sustainability, and working with health systems.  In addition there were opportunities to network with organizations involved with Living Well from across the state.
