What is the Oregon Self-Management Hub?
Evidence-based chronic disease self-management programs, such as Living Well with Chronic Conditions, are innovative interventions that span community and health system settings. They help people take care of themselves, stay healthy, and live better with existing mental and physical health conditions.
In 2012, the Oregon Health Authority worked with many stakeholders to identify a long-term sustainability strategy for evidence-based self-management programs. The outcome of that effort was the Self-Management Business Plan (pdf).
The business plan describes an innovative system - the Oregon Self-Management Hub - that will make Living Well with Chronic Conditions and other self-management programs available statewide long term. The Hub will play an important role in achievement of the Triple Aim of health system transformation: better health, better care and lower costs.
- Program purchasers will gain streamlined, single-point access to a statewide contracting network of organizations that deliver high-quality programs shown to improve health outcomes and reduce costs.
- Program delivery organizations will decrease their administrative overhead, and gain the ability to receive payments via the Hub platform whether or not they can bill insurance. View licensed organizations in Oregon (pdf).
- Oregonians will have access to programs in their communities that help them take care of themselves, stay healthy, and live better with ongoing mental and physical health conditions.
How will the Hub work, and how will it be put in place?
In 2014, OHA plans to release a competitive solicitation (RFP 3720) through the Oregon Procurement Information Network (ORPIN) to identify the organization that will develop the Hub in collaboration with Oregon's self-management program delivery stakeholders.
