Case Management Provider Tools
The Oregon Health Authority, HIV Program has a commitment to increase access and decrease disparity for persons living with HIV and their affected family members. As part of this goal, the HIV Care and Treatment program standardized case management services throughout Oregon in 2001.

Treatment Adherence: The HIV Client Services program recognizes the importance of treatment adherence. The program is committed to providing information and tools to help HIV Case Managers learn more about how they can help clients with treatment adherence issues.

Standards and Forms/Guidance: These standards are designed to provide guidelines which help assure that program funds are spent on services that are in compliance with generally accepted standards of professional care. These standards and forms have been implemented statewide. The Services Definitions & Guidance document provides case managers with valuable definitions, program and reporting guidance on Ryan White Part B eligible services.

Ryan White Part B Reporting Instructions and Forms: As part of standardizing case management services, the program has also standardized its reporting mechanism.

Housing: Equal access to services is a primary barrier for many clients: Get more information on housing opportunities.

Program Eligibility: Client must be HIV positive, enrolled in HIV Case Management, and must define their primary residence as in Oregon. To qualify for Ryan White financial assistance, a client's gross income can be no more than 250% of Federal Poverty Level (FPL).
