Check the “Other Sendout For” box on the Virology/Immunology Request and write in “Leishmania Antibody.”
 
Also complete the CDC Specimen Submission Information form located at: https://public.health.oregon.gov/LaboratoryServices/Pages/CDC-Testing.aspx.
Availability
All clients with approval from the Oregon Public Health Division Acute and Communicable Disease Prevention Program.

PCR and/or Serology performed at the CDC
