Submission of Specimens

All specimens must be submitted following current approved packaging and shipping protocols for diagnostic specimens or infectious substances. All specimens must be labeled with the request bar code label and another unique patient identifier e.g. the patient's full name (first and last), chart number, date of birth. Anything used as the second identifier must also be entered on the test request form so the specimen and test requisition are connected by both unique identifiers. All specimens must be accompanied by the appropriate completed request form.

The legally authorized provider requesting tests completes those areas on the form concerning clinical history. Specimens that OSPHL forwards to CDC for testing must be accompanied by a completed CDC form 50.34. CDC will not perform tests unless every section on the form has been completed. When a CDC test request form is indicated the test is performed at CDC and OSPHL acts only as an intermediary.
