BENTON COUNTY – Registered Nurse Care Coordinator & Public Health Nurse – Benton County Health Services – Corvallis, OR
Registered Nurse Care Coordinator (RNCC) with Community Health Centers:
The RNCC works as a member of a multidisciplinary care team to provide patients a variety of nursing services. This includes care coordination, health education, nursing assessment, patient triage, and medication counseling. Care is provided to clients of all age ranges.

Public Health Nurse (PHN) with Public Health:
The PHN provides nursing assessment, health education, and care coordination with pregnant women, families and children. The PHN may visit pregnant woman and families at the Health Department or may do home visits. The PHN may also participate in community planning meetings or coordination meetings to provide expertise or build partnership with community resources. The PHN also has a role in addressing and preventing communicable diseases and promoting immunizations.

DOUGLAS COUNTY – Communicable Disease Registered Nurse – Douglas County DHS – Roseburg, Oregon
This position is responsible for coordinating and supervising the Communicable Disease Program, HIV Prevention Program, Tuberculosis Program and the Perinatal Hepatitis B Program.  The CD Coordinator coordinates investigation of communicable disease and suspected and confirmed outbreaks under the guidance of the Oregon Health Authority and the Health Officer.  Additional duties include writing policies and procedures, promoting immunizations for vaccine preventable diseases, working with other agencies on local community communicable diseases and immunization issues.  The CD nurse assists in implementation and compliance with other state program elements such as Hepatitis B screening, immunization facilitation and school exclusion and other duties as assigned.

LANE COUNTY – Public Health Nursing Supervisor – Public Health/Maternal Child Health – Eugene, OR

This position’s duties will include, but are not limited to: planning, organizing and supervising nursing and paraprofessional staff in the Maternal Child Health program of Public Health, working collaboratively with the nursing supervisor for the Nurse Family Partnership program and with the Coordinated Care Organization for provision of nurse home visiting services.

MARION COUNTY – Public Health Nurse 1 – Immunizations, Temporary – Marion County Health Department – Salem, OR
This position provides nursing care to individuals in a clinic setting, with a focus on the provision of direct care that promotes health, prevents disease, and treats illness of the individual.  Typical duties:
• Gives immunizations, injections, and skin tests to clients.
• Secures children from moving when giving shots.
• Reads Tuberculosis skin tests and makes appropriate referrals.
• Interviews clients to secure medical history, reviews and evaluates immunization records, forecasts immunization needs and identifies possible contraindications to immunizations.

MULTNOMAH COUNTY – Licensed Practical Nurse – Multnomah County Health Department – Portland, OR
The Licensed Practical Nurse tracks the health of the population of patients assigned to a provider panel. This includes proactively tracking prevention activities, screening and chronic disease monitoring and providing patient education. The LPN provides scrubbing for upcoming provider appointments and participates in huddles. Manages team communication including answering team phone, addressing messages and in-basket as well as patient communication and follow-up. Performs general provider support duties as needed to support clinic flow including rooming patients, ordering labs as directed, giving immunizations, and medications as needed.

MULTNOMAH COUNTY – Clinical Services Specialist, Addictions Benefit Coordinator – Multnomah County Health Department – Portland, OR
The Addictions Benefit Coordinator will work as part of the Addictions Benefit Coordination Team to perform outreach as well as coordinate addiction treatment between levels of care for CareOregon (CO) members whose mental health benefits are managed by Multnomah Mental Health (MMH).  The Addictions Benefit Coordination (ABC) team would provide care coordination and route CO/MMH members to the appropriate level of addiction care and would also help partner agencies and organizations connect high risk members to addiction treatment.  Care Coordinators would work with clients at Hooper Detox, to ensure appropriate referral, proper prioritization for residential treatment, and basic care coordination.
